Application Forms

Below are all the forms that are required for full enrollment into Happy House Daycare. The first form entitled Daycare application form is needed to join our waiting list. The rest are needed once enrolled. The getting to know your infant is only needed for those who are 0-19months. The parental waiver for the playground use is only needed for those ages 3-5. All forms must be filled out completed and then emailed or dropped off to the daycare.
